Overview

Fine-needle aspiration (FNA), a method of aspiration biopsy cytology, continues to grow in use worldwide thanks to improvements in CT and ultrasound imaging. The dominant clinical sites for FNA remain the breast, thyroid, and lymph nodes among superficial tissues.

This lesson covers the clinical skills required to perform FNA, the thin-needle aspiration method and basic equipment, the step-by-step aspiration technique, smear preparation, and the organization of a dedicated aspiration biopsy service.

A patient with a palpable thyroid nodule undergoes FNA. The pathologist obtains only a few drops of material, and the technologist must work quickly and precisely โ€” expelling drops onto slides, compressing and pulling apart smoothly, and fixing some slides immediately โ€” to preserve every diagnostic cell from this limited, irreplaceable sample.

FNA requires familiarity with general anatomy, a focused clinical history, and physical examination skill. The pathologist must be actively involved in both the initial and final evaluation of smears, since clinicians performing the biopsy alone typically lack cytomorphology experience.

Needles of 22โ€“27 gauge, typically 1.5 in. long, are used with an aspiration handle (e.g. Cameco Syringe Pistol), 10โ€“20 mL syringes, alcohol/betadine skin preparation, glass slides, transport media, alcohol spray fixatives, and formalin for cell-block preparation. The thin-needle technique carries virtually no complications except thoracic pneumothorax risk or bleeding in transabdominal aspirations.

The technique involves reviewing patient history, determining biopsy justification, palpating and localizing the mass, positioning the patient comfortably, and obtaining informed consent before aspiration. Afterward, small drops are expressed onto glass slides and either pulled apart horizontally (smear) or vertically (compression/pop smear), with some slides wet-fixed and others air-dried.

A free-standing clinic should be conveniently located on the ground floor within an established medical facility, near referring physicians' offices, with two examining rooms, waist-level counters, and an examining table positioned away from aspiration instruments and the smear preparation area.

The Science Behind This Test

FNA works by using negative pressure (aspiration) through a thin needle to dislodge and collect cellular material from a lesion. The thin gauge minimizes tissue trauma and complication risk while still yielding sufficient cellular material for cytologic evaluation โ€” a 'sample and analyze' alternative to more invasive tissue biopsy.

Step-by-Step Procedure

Review history and justify biopsy

Determine the clinical problem, its relevance to the lesion, and whether the biopsy is justified.

Palpate and plan approach

Localize the mass, estimate depth, and decide the optimal needle direction โ€” perpendicular for deep masses, near-horizontal for small superficial ones.

Position patient and obtain consent

Position the patient so the mass is easily palpable and immobilized; obtain informed consent listing discussed complications.

Perform the aspiration

Insert the needle, apply negative pressure, and sample the lesion using the thin-needle technique.
